In this portal, you can access financial information such as your account balance and the status of any optical orders, and medical records including diagnosis codes, recommended medications, and allergies.

How to Get Started
If you’re new to the portal, please follow these simple steps:
- Obtain login credentials from us here at Jarvis Vision Center following your appointment
- Go to https://revolutionphr.com/portal/welcome.htm
- Enter those credentials in the space provided in the upper right corner
- Click “Login”
This should take you to your online personal health record. If there is any errant or missing information, please contact us to let us know.
